    This is not just in relation to Wales its across England and Wales.

    What is the process to stop this? The Senedd is impotent and incapable of sorting the underlying problems that underpin the failures. The same platitudes are rolled out every time ….”.the Board apologies and says that lessons will be learnt ”

    The lessons have not been learnt and the death toll continues to rise.

    The public needs to thank the Coroner for trying to influence improvements and getting the truth out into the public domain.
